The focus is on the value of causal reasoning and its practical application across high-risk incident themes, including mooring, fall to water, enclosed space entry and SIMOPs involving dropped objects.

It is designed to be interactive and practical, building on our journey with causal reasoning and fail-safe thinking. The materials support meaningful discussion on what causal reasoning maturity looks like in practice, where weaknesses may exist, and how stronger learning can lead to more effective barriers, better operational outcomes and measurable business value.
